1. If you have a strip of bare patch, then chances are it's due to people using that area as a footpath. Grab yourself some pavers or at the very least some stepping stones and turn this area into a proper footpath. You'll never get grass to grow if people are constantly walking on it.
2. Completely dead patches may mean you have curl grubs under the soil surface. Dig under a dead patch of lawn and have a look. If you find pale curled up grubs then a chemical treatment called Baythroid Lawn Beetle Killer will control these infestations. Be careful with this stuff though it's very powerful and can kill birds and pond fish.
3. The lawn has died off where your dog goes to wee. This problem usually occurs with female dogs that are fed a lot of dried dog food. If you feed her fresh meet, uncooked bones and canned food and reduce the dried food, the health of your lawn will improve.
4. Bare patches in shady spots are extremely difficult to fix. You may need to switch to a more shade tolerant lawn variety if the shade is only partial. You'll still have to regularly water, fertilize and not let anyone walk on this area or it still won't do very well. The best bet for a completely shady spot is to get rid of the lawn altogether. Extend the garden bed out into that area and put in some shade loving groundcovers or plants.
